In 1986, Dr. Neppe was recruited internationally from South Africa to establish the first division of neuropsychiatry in a USA psychiatry department (University of Washington, Seattle). Since 1992, he founded and directed a model institute, the Pacific Neuropsychiatric Institute, in Seattle (pni.org). Additionally, Dr. Neppe has served as adjunct professor in the Department of Psychiatry and Behavioral Neuroscience (previously Dept. of Neurology and Psychiatry) at St. Louis University since 1994, director and distinguished scientific advisor in the Human Performance Enhancement Division of the World Institute for Scientific Exploration, and neuropsychiatry consultant for the South African Brain Research Institute. He pioneered adjunctive anticonvulsants in Psychiatry and a successful treatment for the previously incurable tardive dyskinesia. He has also described four new medical conditions, invented approximately 100 new terms, suggested three major new classifications, developed more than 30 evaluations, tests, and historical screens in Neuropsychiatry and Consciousness, and initiated several new theoretical concepts in his disciplines. Further, Dr. Neppe has pioneered the links of brain function and subjective experience, became the world authority on déjà vu phenomena, and was responsible for developing phenomenology in the neuroscience and consciousness contexts. Renowned for his work in numerous areas of medicine, Dr. Neppe has been recognized by peers in all 19 editions of America's Top Doctors and was apparently the only physician listed in 5 different specialties namely neuropsychiatry, behavioral neurology, psychopharmacology, psychiatry and forensics. Importantly, he became the first USA MD Fellow of the Royal Society (of South Africa) in 2002.

His unique contribution has been in 'Dimensional Biopsychophysics' (2011 on), a discipline he pioneered with a new way of conceptualizing the world through the Neppe-Close 'Triadic Dimensional Vortical Paradigm' (TDVP). This proven mathematical model, is also empirically supported, demonstrating nine quantized finite dimensions embedded in an infinite continuity; that reality requires 'gimmel' (likely 'consciousness') necessarily in union with all stable particles, as the third component besides mass and energy; and that TDVP apparently unifies the quantal, macroworld, and cosmological under one law. TDVP also suggests 'Unified Monism' as a mind-body philosophy; evolutionally, postulates 'Meaningful Evolution'; extends Popperian scientific falsification to Lower Dimensional Feasibility Absent Falsification (LFAF); and links science and spirituality.

An international speaker, author and playwright, Dr. Neppe has delivered 1200 invited scholarly lectures and presentations, and has authored more than 800 scholarly articles in his field, including 11 books, 2 plays, and contributing chapters to 64 books. His books include "Cry the Beloved Mind: A Voyage of Hope," "Innovative Psychopharmacotherapy", 4 Déjà Vu books, and "Reality Begins with Consciousness: A Paradigm Shift That Works." 5 Editions) (Book of the Year Finalist). In 2002, he authored a play called "Quakes," and in 2006, he contributed to the movie trailer "Déjà Vu". In 2017, Dr. Neppe became editor-in-chief of the Journal of Psychology and Clinical Psychiatry.

He obtained the Bachelor of Medicine and Bachelor of Surgery in 1973, a Diploma in Psychological Medicine in 1976, Master of Medicine in 1979, and Doctor of Philosophy in Medicine in 1981, at the University of Witwatersrand in Johannesburg, South Africa and was medically trained there. He concurrently received a Bachelor of Arts (University of South Africa, 1977). He acquired his Doctor of Medicine in the United States in 1982, and was a Neuropsychiatry, Behavioral Neurology and Psychopharmacology Fellow at Cornell University in New York between 1982 and 1983. In the USA, he's been Board-certified by the American Board of Psychiatry and Neurology (1988), Geriatric Psychiatry (1991 & 2001); and Forensic Psychiatry (1994); in Behavioral Neurology/Neuropsychiatry (2006, USCNS), and by American Board of Psychological Specialties in Psychopharmacology, Psychiatry and Neurology (2006). In South Africa, he obtained the FFPsych(SA) 1977) and in Canada, the FRCPC (1987).

Dr. Neppe has affiliated with numerous national and international organizations including: Distinguished Fellow of the American Psychiatric Association (2007); Executive Director for the Exceptional Creative Achievement Organization where he is the only Distinguished Creative Polymathic Professor (2010-Present); International Society for Philosophical Enquiry (ISPE) (Diplomate 2009-, 2008 Senior Research Fellow) an Olympian at the Olympiq Society (2017).

Dr. Neppe (with Edward Close, PhD) unanimously received the worldwide, interdisciplinary Whiting Memorial Prize by the ISPE in 2016 for 'expanding boundaries of scientific understanding'. He also received a Gabino Barreda Award in Mexico (2013), and The Marius Valkhoff Medal (South Africa, 1982) Other distinguished awards include listed by peers under 'America's Top Doctors' (every edition; 2001-2019) at times in 5 specialties and subspecialties; 'Leading Psychopharmacologists of the Twentieth Century' (CINP 2004); and social recognition: 'Bonei Habayit' Award (2017, with his wife, Lis, 2017, CSTL).
