Becoming equally renowned for his extensive research and consulting work, his tenure in medicine also included serving as a representative for the American College of Chest Physicians to the American Registry of Pathology in Washington from 1981 to 1992. As a researcher, Dr. Spagnolo received numerous grants to support his work.

Joining the aforementioned VA medical center in 1972 as the assistant chief of the medical service, Dr. Spagnolo advanced to become the chief of staff, the director of respiratory care and the senior attending physician in pulmonary diseases. Recruited as a faculty member of the George Washington University in that same year, he further flourished as an assistant professor of medicine, a professor and the associate chairman of the department of medicine. He also held instructor positions at Boston University and Tufts University between 1970 and 1971. During his academic career, Dr. Spagnolo notably trained approximately 100 pulmonary and critical care specialists.

Providing his expertise for many years on behalf of The Washington Hospital Center, Dr. Spagnolo likewise consulted for the Will Rogers Institute in White Plains, New York, the U.S. Department of Labor in Washington and the Walter Reed Army Medical Center in Washington. He also worked privately for such individuals as President Ronald Reagan and Pope John Paul after their attempted assassinations as well as for several White House physicians during the Bush Administration. Dr. Spagnolo has notably participated in many radio and television appearances where he has spoken about various health-oriented programs.

Dr. Spagnolo has written extensively in his field, authoring and co-authoring several medical books, book chapters, and scholarly journal articles. Some of his credited books include "Air Pollution and Lung Disease in Adults" in 1994 and "Handbook of Pulmonary Drug Therapy" in 1993. His many published articles have been included in such prominent peer-reviewed journals as the American Journal of Cardiology, Clinical Research and the Journal of Nuclear Medicine. Dr. Spagnolo has also supervised numerous medical journal publications over the years and was appointed to the editorial board for CHEST Journal from 2002 to 2006.

A frequent speaker on pulmonary and critical care medicine, Dr. Spagnolo was an invited lecturer in such locations as the U.S., Russia, and Jordan, and participated in many worldwide medical conferences. Additionally, he received the opportunity to testify before the U.S. Congress on health care policy and quality of care as an expert in his field. Among his many other noteworthy achievements, Dr. Spagnolo was the first to ever review patient outcome during the early history of intensive care units, the first to report of Pneumocystis Carinii Pneumonitis in patients with lung cancer and the first to complete a prospective evaluation of short course therapy in the U.S. utilizing Isoniazid and Rifampin.
To better contribute to his community, Dr. Spagnolo was the former secretary and vice president of the National Association of VA Physicians, for which he has been elected as president since 2006.

Dr. Spagnolo initially concluded his undergraduate coursework at Washington and Jefferson College in 1961 before earning a Doctor of Medicine at Temple University in the Lewis Katz School of Medicine in 1965. He subsequently completed his residency at the VA Medical Center in Boston as a senior resident and chief resident in medicine. He later conducted a Harvard fellowship in pulmonary diseases at Massachusetts General Hospital after committing to two years of service with a U.S. Public Health Service as a officer and lieutenant commander at the Center for Disease Control in Atlanta between 1966 and 1968. Certified through the American Board of Internal Medicine, Dr. Spagnolo is a licensed physician in Massachusetts, Virginia, Maryland, California and Florida.

Dr. Spagnolo was notably selected as a fellow of the American College of Chest Physicians and the American College of Physicians. He is also affiliated with several other leading medical societies, including the American Society of Internal Medicine, the American Medical Association and the National Association of Veterans Affairs Physicians and Dentists. He is also associated with the D.C. chapter of American Thoracic Society's D.C. chapter as a member and former executive committee member.

Nominated for the Golden Apple Award by the George Washington University in 1977, Dr. Spagnolo was later celebrated for his medical expertise with a Certificate of Appreciation by the D.C. Lung Association in 1983. Additionally, he was decorated with a Cavalier Order of Merit by the Republic of Italy. Consistently published in American Top Physicians and Best Doctors in America between 2006 and 2014, Dr. Spagnolo is also a celebrated Marquis listee who has been cited in Who's Who in America, Who's Who in American Education and Who's Who in Medicine and Healthcare.
