Affiliated with Marshall Health, which is one of the region's largest employers, he notably works in the department of internal medicine and has hospital privileges at both Cabell Huntington Hospital and Saint Mary's Medical Center. In addition to overseeing his practice, Dr. Alastal began teaching in 2017 as an associate professor of medicine at Marshall University, a public research institution in Huntington.

Inspired to study medicine because of a lack of health care systems in his native Palestine, he was one of many children to grow up with inadequate medical attention. Dr. Alastal attained a Doctor of Medicine at the University of Hamburg in Germany in 1994. He subsequently completed a residency at Makassed Hospital in Jerusalem, followed by both a residency and fellowship at the Medical College of Ohio in Toledo. Dr. Alastal is board certified in critical care medicine through the American Board of Internal Medicine, holds certification in pulmonary disease, critical care medicine and sleep medicine, and is licensed to practice medicine in both West Virginia and Ohio.
Dr. Alastal was elected as a fellow of the American College of Chest Physicians. He is also a prominent member of numerous other industry-related organizations, including the Society of Critical Care Medicine, the Jordanian Medical Association, the American Medical Association, the American College of Physicians, Palestinian Physicians Syndicate, and the American Thoracic Society, and is an honorary member of the Palestine Medical Council. Recognizing his achievements, Dr. Alastal was named as the Best Physician in Dubai for 2011. He was also recognized as the Best Physician in Palestine for two years in a row.
