With over 40 years of recognized excellence in medicine, Dr. Mahnensmith continues in direct care service as Primary Care Physician for individuals and families with the Primary Care Medical Group of DOCS of CT. As a Primary Care Physician, Dr. Mahnensmith aims to connect meaningfully with each patient and provide personalized, holistic care to his many patients, honoring God and God's Calling to Serve. His basic nature is to listen carefully, follow up with focused questions regarding symptoms and concerns, and then make shared decisions with his patients regarding specific treatment options. He is knowledgeable and experienced in the diagnosis and care of a broad spectrum of medical disorders including many kidney, nutritional, and metabolic disorders, diabetes, hypertension, lipid and cardiovascular disorders, and varying infectious ailments. He feels humbly "Called" to the care of people who are variously afflicted and aims to connect with each person with caring and comfort, helping them and their families cope with their varying medical problems over time.
Highly-esteemed, Dr. Mahnensmith has been repeatedly rated five-stars by his patients for expertise, compassion, and trustworthiness. "Dr. Rex might be the best doctor I've ever encountered," reads a patient review. "His thorough knowledge and dedicated compassion in his care is amazing. I've never had a doctor who connected, cared, informed, and really tried to understand my situation more than Dr. Rex. It's clear that being a physician is this man's True Calling!"
Dr. Mahnensmith's dream of becoming a doctor of medicine resulted from a debilitating injury he sustained while pole vaulting in a Track and Field Meet in high school. Unable to walk for many months, he recalls how he reflected on the care he received from local physicians and surgeons in his small hometown of Bluffton, Indiana on the banks of the Wabash River and hoped someday to help others as he was helped.
His dream grew while at Denison University in Ohio, where he majored in Biology while completing premedical studies. He was selected into the Phi Beta Kappa Honorary Society as a junior student plus Alpha Omega Alpha, Alpha Phi Omega, and Omicron Delta Kappa and he graduated Summa Cum Laude in 1973. He received a scholarship to Yale School of Medicine and graduated Cum Laude in 1977. He was singularly recognized as the recipient of Yale's Berniker Award, an award presented at graduation ceremonies to the individual most exemplary of the Oath of Hippocrates and Prayer of Maimonides. He realized his own special "Call" that year.
During his residency in Internal Medicine at Yale-New Haven Hospital in New Haven, CT, Dr. Mahnensmith was recognized with The Kushlan Award, a recognition awarded by the Yale Internal Medicine Faculty as the resident who exhibited the best bedside manner and devotion to the care of patients. He was selected to serve as Chief Resident, which tracked him into a combined clinical care-teaching in Academic settings.
From 1984 through 2014, Dr. Mahnensmith served as Clinician-Educator first at Duke Medical Center, then Brown University program of Medicine, and finally back at Yale, where he rose to the rank of Full professor of Internal Medicine and Clinical Director of Nephrology. In each program, Dr. Mahnensmith was recognized repeatedly as Teacher of the Year, Model Physician, and Exemplary Clinician-Educator. Dr. Mahnensmith precepted and taught medical students, nursing students, physician-associate students, interns, residents, and advanced fellows in various settings: bedside, various seminars, formal classroom settings, and in Yale's Master's program, and he relates that he regularly emphasized one's "Call" to serve others. At both Brown and Yale, Dr. Mahnensmith introduced and guided innovative care methodologies with dialysis and metabolic care pathways.
After serving these 30 years as both Educator and Consulting Physician, Dr. Mahnensmith formally retired from Yale with Full professor Emeritus Honors and moved to live in a small community in Connecticut so that he and his wife could provide daily help with their young grandchildren. At the same time, Dr. Mahnensmith could not remain uninvolved with patient care, so he transitioned to serving elderly patients as a Primary Care Internist with three nursing homes in Northern Connecticut. Families adored him and sought him for care of their loved ones. With his positive reputation among these communities, he was recruited to serve as Quality Assurance Leader in a Federally Qualified Community Healthcare Center in Waterbury, CT – The Stay-Well Health Center. He also took on Primary Care there and eventually served as Chief Medical Officer. He retired from these roles in 2017 to spend more time with his family. And then, he received another recruitment phone call from DOCS of Connecticut, again to serve in a needed Primary Care Role and additionally to take on Preceptor Roles for Physician Assistants and Nurse Practitioners within the DOCS Group.
Dr. Mahnensmith's unique excellence with clinical teaching is immeasurable; he shares that he deliberately aims to connect and take extra time to instill a strong sense of "Vocation" – what he calls the "Calling" to serve others with care and carefulness. It is a humble privilege and a servant-role, he continually taught and continues to model today.
While he served on the Yale Faculty, Dr. Mahnensmith was especially selected to serve as Medical Director for two free-standing community dialysis clinics in New Haven and Branford, Connecticut. In these roles, Dr. Mahnensmith led safe practices, directed quality improvement activities, and implemented many original patient-care-oriented Clinical pathways for special clinical problems prevalent in the dialysis setting: Blood Pressure care, Infection Control, Anemia Management, Nutritional Optimization, Bone Health Maintenance, and ensuring the Adequacy of Delivered Dialysis. These unique Clinical pathways received national recognition and were adopted and implemented by two national dialysis corporations. In response, he was invited to serve as an Advisor to National Medicare Committees who were composing Dialysis oversight and he accepted and served in this capacity for three years. He also was selected as Regional Medical Director for two National Dialysis Corporations. In this capacity, he led and taught many physicians and head nurses toward Best Practices. And, he was awarded American Society of Nephrology Master Award in 2020. Dr. Mahnensmith attributes his career-successes to caring educators, the enduring love of his wife and family, and abiding relationships with his patients. His energy and humble perseverance derives from his clear sense of his "Call to Serve," and he aims always to honor his Call in all that he does, with humility and caring, which was truly recognized by Yale Medical School Faculty in 1977 at his graduation ceremony.
