He continued with this hybrid role for two years before progressing to his current appointment as a faculty pharmacist with St Peter Family Medicine in 2018.
As a pharmacist, Dr. Lancaster supports primary care physicians and thrives in collaborative care models. He is a critical member of the clinic's complex case review team, a multidisciplinary team comprising a pharmacist, behaviorist, physical therapist, nurse practitioner, and nurse care navigator. The assembled experts discuss patients at high risk for readmission weekly and have contributed to a significant reduction in hospital readmission rates since the team's inception in 2016.
